West Chester EyeCare has permanently closed.

 

TO REQUEST RECORDS:

To request copies of your records, please send an email to recordswceye@gmail.com.  Please include your name, date of birth, and where you would like the records sent.  They can be emailed or mailed.  In the near future, fax will also be an option.  Records for Feb 2014 to Feb 2018 are electronic, and can be sent within a week (sometimes quicker).  Records prior to Feb 2014 are paper.  They are being securely stored, and will require some time to retrieve.  Please allow up to 4 weeks.  Please indicate in your request if you want records older than 2014 to be included.  Also indicate if you had any extra testing that you would like included.  Any digital imaging will need to be emailed.

 

IF YOU WOULD LIKE TO CONTINUE TO SEE DR. BEREDA:

If you would like to continue to see Dr. Bereda for your eye exams, she is now working at Family Eye Care of Ohio, which has offices at three Lenscrafters locations.  Dr. Bereda works at the Monroe and Mason offices.    There are a few doctors working at each location, so be sure to request her by name when scheduling an appointment.  Also, please send an email to recordswceye@gmail.com to let her know you have scheduled an appointment.  This will give her a chance to look over your previous records.   Contact info for Family Eye Care can be found at familyeyecareofohio.com.

 

ORIGINAL ANNOUNCEMENT:

It is with a heavy heart that I announce that effective February 23, 2018 I will be closing my practice. West Chester EyeCare will no longer be able to take care of your eye care needs after that date.

Changes in the industry, along with stagnant or decreasing insurance reimbursements, have made it difficult to maintain an independent optometry practice. In addition, I was diagnosed with cancer in October. I had to take time off for surgery, recovery, additional testing, doctors appointments, and ongoing treatment. This was a huge setback, and key factor in the decision to close.  My prognosis is good, and I should be okay after completing a year of treatment.

If desired, you may call the office to obtain copies of your records. Please allow us several days to get them ready for you, as we have had numerous requests. Records will also continue to be available after we close. I will be posting more info within the next month with instructions on how to request copies of your records after our closing date.

It has been my pleasure to take care of your vision needs for the past 20 years. You are the reason I enjoyed coming to work every day.
